At your first session, you will be asked to sign our client acknowledgement form which states you have read and/or reviewed the following documents relevant to your treatment and rights as a client at our clinic. You will also be given the opportunity to ask questions regarding these documents:
- Informed Consent
- Privacy Practice Notice
- Clients Rights and Grievance Procedure
- Clinic for Christian Counseling, LLC Mission Statement and Statement of Beliefs
Your signature on the client acknowledgement form will also indicate that you consent to enable our clinic staff to use and disclose your personal health information for purposes of treatment, payment and health care operations.
